It starts at however long it takes anyone to reach them at all — and in the mountains here, that can run to hours before someone with training arrives, then another hour to a facility.

On-site standby removes that gap completely. The paramedic is already there, with the monitor, the airway kit, the medications, and the training to use them. Treatment starts in the first minute instead of the hundred and twentieth, and the call on whether someone needs evacuating is made by a clinician who is actually looking at them.
This is ordinary practice for events and worksites in most of the world, and it costs a small fraction of what one bad outcome costs. Weddings, tournaments, charter boats, hiking and mountain-biking groups, construction phases, film production, festivals — anywhere you have gathered people somewhere help cannot reach quickly.
Fishing tournaments and charters deserve a particular mention. Your guests are often older, frequently managing a condition or two, and hours from a dock with no way to shorten that. A paramedic on board changes what is survivable out there — and it is the difference between a story your guests tell for years and one nobody wants to tell at all.
